We will not be collecting personally identifiable information beyond what is included in the survey questions themselves. However, we ask you to refrain from including personal data in the free text boxes of the survey.

By taking part in this survey you:

• Authorise the ICO to process your personal data according to the terms in the privacy policy for the purpose of completing this research study, including the use of sub-processors listed in the privacy policy
• Authorise the ICO to process your special category personal data, if you choose to share it, to understand user experience and make improvements to our website
• Authorise the ICO to process your name and email address, if you’re interested in taking part in future research, for the purposes of adding you to our list of volunteers and inviting you to participate in future research when the opportunity arises
