Wellbeing in Adolescents during the current Coronavirus (COVID-19) - Parental Consent

1. Welcome

Thank you for your interest in our research project related to wellbeing in adolescents during the restrictions due to the Coronavirus!

This project is part of a study at the University of Birmingham trying to better understand the relationship between stress, physical activity, and mental health and wellbeing in adolescents during the current Coronavirus (COVID-19) pandemic.

We hope you have had access to a copy of the information sheet, either from your child if they have had this sent to them by their school or club, or through the recruitment notice that directed you to this consent form.

If you have read the information sheet, and your child is aged 13-15, please press next to give consent for your child to take part in our study.

If you have not read the information sheet or would like to read it again, please follow the link below:

If you have any questions related to the study, please do not hesitate to contact us.

Thank you very much for your support for our research,
