The purpose of the Young Dementia Network is to improve the lives of people with young onset dementia and their families now and to create positive change for the future. We are reviewing the Network's progress and developing a strategy that will set out what we want to achieve from now until 2028.
We are inviting you to complete and return this short survey to help shape the Network’s focus for the next three years. The survey should take around 15 minutes to complete and we would really appreciate your input. The survey closes on Friday 21st February 2025.
This survey will not ask for any personal identifiable information and is designed to be anonymous. If you provide any information that identifies you or another person in your answers, we will delete this information before using your answers to help inform our strategy.
