Healthwatch Lancashire are looking into the process of hospital discharge at Lancashire Teaching Hospitals, this includes both Chorley Hospital and Royal Preston Hospital. We want to speak with people who have recently been discharged in the last year and ask them their thoughts and feelings regarding how they felt during the process and what could be changed.
This project is in response to recent public feedback around the discharge process and the understanding of what Hospital discharge is and how this is communicated to patients.

All responses will be anonymous. To protect your anonymity, your individual views will be combined with those of other respondents and analysed at a combined level. Healthwatch Lancashire will hold all information given, remain secure and confidential, and will be used in line with the Data Protection Act 2018.
