King's Hair Loss Service - Patient Feedback Survey

Page 1

The King’s College Hospital NHS Foundation Trust cancer patient experience programme team are keen to improve the quality of patient care for cancer patients as a priority – that means listening to and acting on your concerns.

Following your experience of using the King’s hair loss service, please take 5 minutes to complete this short anonymous survey to provide us with feedback. We will use your responses to evaluate and continue to improve our service, to help us to provide quality care for patients.

It would be very helpful if you could complete the survey but if you choose not to, this will not have any impact on your current treatment or in accessing future services.

We offer other opportunities to help us deliver on improvements to patient experience for those who have been diagnosed or treated for cancer at King’s. If you have any questions or would like to get involved, please email us on kch-tr.macmillan1@nhs.net.
