We would love to hear about your experience of using HomeFirst services. These Services include the Virtual Ward, Urgent Community Response, Recovery At Home team and planning of care to leave hospital. Luminus are supporting HomeFirst to better understand:
- The experiences of carers supporting patients using HomeFirst services
- What opportunities there may be to ensure carers feel supported and involved when HomeFirst services are caring for partners, family members or friends.
HomeFirst is a Surrey Downs Health and Care (SDHC) service that cares for people within their homes and communities, or supports them when being discharged from hospital, to help them regain as much independence as possible and reduce the risk of hospital admission. This helps people recover and stay well in their own surroundings.
The service is delivered by a multidisciplinary team including community doctors, nurses, physiotherapists, occupational therapists, rehabilitation support workers, paramedics, pharmacists and advanced clinical practitioners. They also work closely with relevant community and voluntary sector services. HomeFirst is made up of different services all with a shared focus to support people within Surrey Downs to live as independently as they can - so they either stay well enough not to have to spend time in hospital, shorten their hospital stay or gain their independence back following a stay in hospital.

Responses to this survey are anonymous. The closing date for the survey is 22 March 2026.
