ADHD Adult Survey

About this survey

NHS South West London is reviewing how ADHD services are delivered for adults. We want to ensure that people are able to access the right support at the right time — both during assessment and afterwards.

This survey asks about your experiences that led you to decide to seek an assessment for ADHD, accessing information and support, waiting for services, and receiving treatment. It also asks for your views on how ADHD services could be organised and improved in future.

Your feedback will help inform the development of ADHD pathways, including how people are supported while waiting and after diagnosis.

The survey should take around 10–15 minutes to complete. All responses are anonymous and will be used for service improvement purposes. The survey is aimed at people over the age of 18.

The deadline for completing the survey is Sunday 12 April 2026. 

How we will use what you tell us 

Any confidential information you share in this survey will be anonymised and will be used to help inform how these services are run. If you have any questions about this work, or need support completing the form, please contact: us on  020 4526 2858 (Monday - Friday 0900-1700).
