This form is used to gather professional medical advice to inform a Section 19 (Education Act 1996) decision regarding a child or young person’s ability to access full-time education due to health needs.
Parent/carer consent has been obtained by the school for you to share relevant medical information.
When completing this form, it is important to distinguish between ongoing health needs and longer-term special educational needs and/or disabilities (SEND). While these can often overlap and interact, Section 19 provision is specifically intended to address situations where a medical condition is the primary barrier to accessing education in the short to medium term.
