A public survey about the experience of the Patient Advice & Liaison Service (PALS) in Kingston upon Hull, East Riding of Yorkshire, North Lincolnshire and North East Lincolnshire.
This DOES NOT include the PALS services from Humber Teaching NHS Foundation Trust, York and Scarborough NHS Teaching Hospitals and Tees, Esk & Wear Valleys NHS Foundation Trust (TEWV).
This survey is conducted by Healthwatch Kingston upon Hull, the legal Data Controller for this information. We will use your feedback to understand how well the Patient Advice & Liaison Service (PALS) is working and to help local NHS services make improvements for patients and their families.
We will not be asking for any personal data and all results will be combined together for final reporting. We do not explicitly ask for identifiers like your name or date of birth. This survey is hosted on SmartSurvey, which securely processes data in the UK. All information is managed under strict confidentiality.
Your responses will be kept securely and then permanently deleted. Combined, fully anonymised findings will be published in public reports and shared with local NHS partners.
