Dear Patient,
You were recently offered a same day/next day urgent appointment by your GP practice but did not attend. These appointments are additional to those offered during normal practice hours to patients whose health concerns don't require emergency hospital treatment but who have been triaged as needing to be seen urgently by a health care professional.
The Surrey Heath Hub (delivered by local GP surgeries working in partnership) would love to hear your feedback about this, and so have asked us, Healthwatch Surrey, to conduct a short survey on their behalf.
We are seeking to understand the reasons why you didn't attend your appointment and, if you chose to go elsewhere, why was this the case. We would be very grateful to hear your reasons. The survey is anonymous and any information you provide will not impact your future health care with us.
Healthwatch Surrey champions the voice of local people to shape, improve and get the best from NHS, health and social care services. We are independent and have statutory powers to make sure decision makers listen to the experiences of local people.
The closing date for the survey is 25 September 2026.
5 people who complete the survey before 25 September 2026 will be selected at random to receive a £10 All4One voucher. Those selected will be notified within 1 month of the close of the survey.
There are up to 12 questions (plus 12 optional demographic questions), and it will take about 10 minutes to complete.
