Who is Healthwatch Norfolk?
Healthwatch Norfolk is the independent voice for patients, service users and carers in the county. We gather your views of health and social care services to ensure they are heard by the people in charge.
What is this survey about?
We want to find out how long people with sight loss are waiting for diagnosis and support and if they are given any support while waiting
The survey will take 5 to 10 minutes to complete. Some questions are optional.
If you live in Norfolk and would prefer to do this survey with us over the phone, please call Healthwatch Norfolk on 0808 168 9669 and we will arrange a time to ring you back to complete the survey.
Alternatively, please email: enquiries@healthwatchnorfolk.co.uk to receive the survey in alternative formats, including Easy Read, large print , in different languages or audio format.
How the survey results will be used
Survey responses are being collected and analysed by Healthwatch Norfolk . You can read Healthwatch Norfolk's privacy policy at: www.healthwatchnorfolk.co.uk/about-us/privacy-statement.
All responses will be anonymous. The survey findings will be used as part of an independent evaluation of the support for people with sight loss. The report will be publicly available on our website and may be used in other Healthwatch Norfolk communications.
