About this survey
Have you been diagnosed with dementia and live in Birmingham or Solihull? Or do you care for someone with dementia who lives in Birmingham or Solihull?
If so, we would like to hear about your experience of the support available after a dementia diagnosis.
We are carrying out this survey to understand whether people are getting the information, advice and support they need following a dementia diagnosis. Your experiences will help us identify what is working well, where people need more support, and what could be improved.
The survey should take around 10-15 minutes to complete.
All information will be treated confidentially and stored securely. We will not publish any personal information that could identify you.
