Sustainable Communities
Rural Health and Care Wales (Hywel Dda University Health Board, Betsi Cadwaladr University Health Board and Powys Teaching Health Board) is looking to support two local communities in Ceredigion through the Sustainable Communities project (RIF funded) that will run weekly social activities for residents and facilitate the provision of support for them to live independently, in their own homes, for as long as possible. The project’s intention is to help build a community-led solution to increasing community cohesion and wellbeing, so that the work can continue if participants feel it is beneficial. The support of each community and the community groups that already exist in these locations is therefore essential.
We would be grateful if you could please complete the following short survey so that we can explore whether the proposed project would be welcome in your community and how we could work with you to make it a success. There is some funding available to hold events and hire community premises in your community as part of the project. We will support the communities during the project period and look to establish a sustainable model of support for local people now and in the future.
We would be very grateful if you could therefore complete the following short questionnaire and thank you in advance for your time.
