About Healthwatch Hertfordshire
Healthwatch Hertfordshire is the independent champion for people who use health and social care in the county. We gather people’s experiences of local services and use them to make recommendations for improvement to the organisations that run them.
About this survey
Adult Social Care Services is responsible for ensuring that residents and family carers feel listened to, respected, informed and actively involved in decisions about their care. This survey asks about your experience of care and support in Hertfordshire, from first contact through to any care and support needs assessment and what happened afterwards. This includes being referred, having any assessment, review or conversation about any care and support needs.
Care Act assessment is any assessment, review or conversation about care and support, whether that is about your own needs, the needs of someone you care for, or your needs as a carer. It might have been called a social care assessment, a needs assessment, a carer's needs assessment, a "Connected Lives" conversation, "the social worker's visit", or simply "being assessed for help".
