You are being invited to participate in a research study titled An exploratory study investigating the recent developments and future of veterinary physiotherapy in the UK. This study is being done by Megan Boden from Writtle University College.
 
The purpose of this research study is to explore the recent growth of the veterinary physiotherapy industry and discuss approaches to overcome barriers in the way of the profession developing and will take you approximately 10 minutes to complete. 
 
“All information obtained will be held by Writtle University College and may be used to inform other studies carried out by the University College. Any enquiry regarding Data Protection should be addressed to the Data Protection Officer”
 
Informed consent questions:
 
1.    I agree to take part in the above research.  I have read the Participant Information Sheet for the study. I understand what my role will be in this research, and all my questions have been answered to my satisfaction.
2.    I understand that I am free to withdraw from the research at any time, without giving a reason.
3.    I am free to ask any questions at any time before and during the study.
4.    I understand what will happen to the data collected from me for the research.
5.    I understand that quotes from me will be used in the dissemination of the research.
6.    I understand that the interview will be recorded.
7.    I agree to the University College processing personal data which I have supplied. I agree to the processing of such data for any purposes connected with the Research Project as outlined to me in the Participant Information Sheet.
8.    I understand that my data may be stored in areas where General Data Protection Regulations (GDPR) do not apply (e.g. servers for Google and Survey Monkey are in the USA)
