Experiences of receiving support from adult social care in Norfolk

Introduction

This survey is for people currently receiving support from adult social care in Norfolk. Adult social care is support that helps adults who may need extra help because of disability, illness, frailty or other circumstances to live as independently as possible. It can include things like help at home, personal care, supported living, day opportunities, equipment or adaptations, direct payments, short-term reablement, or support from a social worker. It can also include advice and support for unpaid carers.

What is this survey about?

Healthwatch Norfolk is working with Norfolk County Council and Norfolk Care Association on the Norfolk Neighbourhood Learning Partnership (NNLP). The project is exploring how adult social care can work more locally, prevent problems earlier, connect people with community support, and provide more joined-up, person-centred help. This survey asks about your current experience of care and support, so we can compare this with people’s experiences as new neighbourhood approaches develop.

The survey will take around 5-10 minutes to complete, although all the questions are optional.

If you have extra feedback you’d like to give, please call Healthwatch Norfolk on 01953 856029, and we will arrange a time to ring you back to gather your views. Alternatively, please email: enquiries@healthwatchnorfolk.co.uk for further support.
