Your views on the future of care and support in your local area

Introduction

This survey is for Norfolk residents who want to share their views about how services and support should work in their local area in the future.

What is this survey about?

Healthwatch Norfolk is working with Norfolk County Council and Norfolk Care Association on the Norfolk Neighbourhood Learning Partnership (NNLP). The project is exploring how adult social care can work more locally, prevent problems earlier, connect people with community support, and provide more joined-up, person-centred help.

Health and care organisations sometimes use the word “neighbourhood” to describe a local area in which services and community organisations work together. In this survey, we use “local area” to mean the place where you live and the nearby places you use for services, support, activities or social contact. This survey asks about your current experience of life in your local area, and what you think services should look like in the future.

The survey will take around 5-10 minutes to complete, although all the questions are optional.

If you have extra feedback you’d like to give, please call Healthwatch Norfolk on 01953 856029, and we will arrange a time to ring you back to gather your views. Alternatively, please email: enquiries@healthwatchnorfolk.co.uk for further support.

Who is Healthwatch Norfolk?

Healthwatch Norfolk is the independent voice for patients, service users and Carers in the county. We gather your views on health and social care services or charities to ensure they are heard by the people in charge.

How will the survey results be used?

Survey responses are being collected and analysed by Healthwatch Norfolk. You can read our full privacy policy at: www.healthwatchnorfolk.co.uk/about-us/privacy-statement.

All responses to the survey are anonymous, so nobody will know what you have said. The information will be used by Healthwatch Norfolk to make recommendations to adult social care on how services can be improved across Norfolk.
