We appreciate your input which will help NHS Scotland to understand the patient perspective around KIS and what further developments you would like to see.

Responses should only be provided by patients with experience of anticipatory care plans, either having created one with their GP or experience of supporting another patient in the process. If this does not apply to you please answer no at question 1 and leave the other questions blank.

We would also welcome patient stories giving examples where having access to KIS is judged to have improved outcomes for patients and carers. Please do not provide any patient identifiers.
