Greetings
I am a undergraduate student studying Product Design and Technology at Loughborough University. I am currently doing research for my final year project of a guitar aid for sufferers of rheumatoid arthritis.
Please read through the participant information and informed consent question before continuing.If you select No to the participant agreement question (below) please do not participate in the survey.
This investigation is carried out by myself (l.j.ingham-13@student.lboro.ac.), and supervised by Abby Paterson (a.m.paterson@lboro.ac.uk) and Steve Summerskill (s.j.summerskill2@lboro.ac.uk)
This survey is to help me identify the problems and possible solutions associated with rheumatoid arthritis in guitar players.
The following survey should take no longer than 30 minutes,and you will be asked to contribute as much or as little as you feel comfortable doing. You have the right to withdraw at anytime and have any data removed from the study. However, once the results of the study have been submitted (25th May 2016), it will not be possible to withdraw your individual data from the research.
The results of this survey will be kept anonymous, and will be stored upon smartsurvey's website as well as in my project logbook. The data I collect will be kept until the end of this academic year, smartsurvey will keep it according to their data protection policy.

1. The purpose and details of this study have been explained to me. I understand that this study is designed to further scientific knowledge and that all procedures have been approved by the Loughborough University Ethics Approvals (Human Participants) Sub-Committee.

I have read and understood the information sheet and this consent form.

I understand that I am under no obligation to take part in the study.

I have had an opportunity to ask questions about my participation.

I understand that I have the right to withdraw from this study at any stage for any reason, and that I will not be required to explain my reasons for withdrawing.

I understand that all the information I provide will be treated in strict confidence and will be kept anonymous and confidential to the researchers unless (under the statutory obligations of the agencies which the researchers are working with), it is judged that confidentiality will have to be breached for the safety of the participant or others.
