Derbyshire Mind - Community Wellbeing Activities and Groups ('Enjoying Derbyshire')
Thank you for completing this survey which has been designed to gather the views and opinions of people living in Derbyshire about what they find most useful from community wellbeing groups and activities. Your answers will help shape the programme of community services Derbyshire Mind offer in the future.
This questionnaire is for anyone living in Derbyshire aged 18 or over, with personal experience of mental health problems either currently or in the past 12 months.
The survey findings will only be shared within Derbyshire Mind in order to help shape our Enjoying Derbyshire strategy going forwards. Your answers will remain anonymous and we are not asking any personal or identifiable details about you.
