Primary Care Networks (PCNs) are groups of GP Practices working together as a network, in which they are commissioned to deliver Enhanced Services. PCN5 is made up of St Georges Medical Centre / The Phoenix Practice / Azim & Partners / Pennine Drive Surgery / Greenfield Medical Centre / Ravenscroft Medical Centre & Hillview Surgery. These services are provided by the 7 practices within the PCN, over and above the core 8.00am - 6.30pm contract. From 1st October 2022, PCNs have been asked to undertake patient engagement, to ensure the new service meets the needs of the local population.
