- Affects a public service or public-facing organisation
- You believe it represents a systemic or recurring problem
- The concern is about risk, harm, failure, or unfairness or unlawfullness
- The issue has not been resolved through normal internal routes
- You think others may be experiencing the same problem
❌ This is not the right place if:
- It’s a personal grievance or employment dispute
- It’s a one-off disagreement with a colleague or manager
- You are seeking individual resolution or legal advice
- You want the charity to investigate or accuse specific people
- You are submitting the same information again without new deta
Our ability to analyse and escalate concerns is primarily focused on the UK and Ireland. Concerns outside this area may be logged for learning but may not be escalated.

- Repeat submissions without new information are grouped together
- Action is taken only when concerns indicate a credible systemic risk
