Within our Equality, Diversity and Inclusion Policy we recognise that it is important to monitor the effectiveness of our engagement with communities to help us identify and address any barriers faced and ensure that our programmes are as inclusive as possible.

We are therefore requesting that you complete this form and return it with your application. The form will be separated from your application on receipt and will not be provided to any member of the selection panel. All information supplied will remain confidential and be held securely and deleted six months after the completion of the IGNITION Project in 2022.

Completion of this form is voluntary and all questions are optional. You are not obliged to answer any of these questions but the more information you supply, the more effective our monitoring will be.
