Vaccination Attitudes Study
Thank you for taking the time to complete this survey. You are one of a small number of participants recruited to share your attitudes about vaccination. We know this is a controversial topic, but we are trying to better understand how people respond to information both for and against vaccination. We are not promoting any perspective on vaccination.
Your participation in this survey is voluntary; however, your participation is important to help us better understand attitudes about vaccination. Findings from this project will be used by the academic team for professional research presentation and publication.
This shouldn’t take more than 15 to 20 minutes of your time to complete and is best completed all at once. This survey is completely anonymous. We have no way of identifying you or connecting you to this questionnaire. This questionnaire will ask you about your own background, your beliefs about different issues, you will be given information about vaccinations and then asked to evaluate that information and the potential sources of that information.
The results of this project will only be reported in aggregate (no person’s response is singled out) and the information will be used by the project team to support an undergraduate dissertation as well as presentation at conferences and publication in peer-reviewed academic journals.
There are certainly no right or wrong answers and we’re only interested in your honest opinions. By responding to the questionnaire, you indicate that you consent to participation on these grounds.
If you have any questions or concerns about the survey, please don’t hesitate to contact Ella Marett-Gregory email@example.com or Dr. Audra Diers-Lawson at firstname.lastname@example.org. This contact information will be repeated at the end of the survey.
