How you can help
We would like to invite you to complete a short survey to understand if you would use an online application to share, view and update what matters to you.

Knowing what matters to you helps us to understand your priorities and needs, and how best to support you to achieve them.  We therefore recognise the need to start with what matters to you, whether you are trying to maintain good health and wellbeing, managing long term conditions or living with complex chronic conditions. 

Knowing what matters to you supports our conversations to ensure we work together to decide what action is most suitable for you.  In turn, this will enable us to tailor our approach to each conversation, using the time in a way that best supports you and your current needs. It allows us to focus on what will be of most benefit to you and what will help you to live a life that is meaningful and fulfilling for you.

The aim in Leeds is to enable you to answer the 6 ‘What Matters to Me’ questions and also update these at any time using an online application called Helm.

Helm is a person held record that gives you the ability add information to your health and care record and provides you with health information and advice. You can find out more about Helm and how to sign up here.

We wish to include the following what matters to me questions within Helm.

1. What Matters to Me?
This could include meaningful activities such as leisure, visiting places, sport and exercise, employment; education and volunteering.  You could also include your religion, values and things you like to talk about.

2. Who are the important people in my life?
This could include any support network you may have, family members, carers, and friends and why they are important to you.

3. What things do I do to keep well?
The things you currently do to keep yourself well. This can also include your strengths and skills, how you engage with others and how you feel on a typical day.

4. Please do
The preferences you many have, for example ‘remind me to take my medication’, ‘encourage me to wash my hands regularly’; ‘explain to me what is happening and why’. 

5. Please don’t
This might include not asking questions about certain topics, not making assumptions about something, or providing support when it is not wanted.
