Gwelir yr holl ddogfennau, ffurflenni perthnasol a fwy o wybodaeth am ein medalau a gwobrau ar ein gwefan.
Pethau rhaid i chi eu gwneud cyn i chi ddechrau'r ffurflen hon:
- Er mwyn cwblhau’r ffurflen enwebu, efallai y bydd yn ddefnyddiol i chi ofyn am CV eich enwebai ymlaen llaw (enwebiad unigolion yn unig)
- Rhannu'r dolen i'r 'Ffurflen Ganiatad' gyda'r unigolyn / cynrychiolwyr tîm. Rhaid i bob ymgeisydd / tîm gadarnhau eu caniatâd i gael eu henwebu cyn i chi gyflwyno’r ffurflen hon.
- Rhannu'r dolen i'r Ffurflen Geirda i'r cyfeirnod a ddewiswyd. Os nad yw'r cyfeirnod wedi cwblhau'r ffurflen erbyn 12:00pm ar y 1af o Fehefin, ni fydd yr enwebiad yn cael ei ystyried.

Nomination Form - Learned Society of Wales Medals and Awards 2026. Nomination Deadline: 12:00pm 1 June 2026

Things you need to do before you start this form:
- You may find it useful to request your nominee’s CV in advance (individual nomination only).
- Share the link to the Consent form with the candidate/ team representative. You will not be able to submit this form until the candidate(s) have completed the Consent form.
- Share a link to the Reference form to the chosen referee. If the referee has not completed the referee form by 12:00pm on the 1st of June, the nomination will not be considered.
