Barking and Dagenham, Havering and Redbridge Clinical Commissioning Groups (BHR CCGs) are currently reviewing the Community Equipment Service and want feedback from service users/carers about their experience of the existing service and where improvements can be made. Community equipment may have been ordered for you by an Occupational Therapist, Physiotherapist or other healthcare professional (e.g. pressure relieving mattresses, ventilators, postural support chairs). Please complete this survey if you have accessed Community Equipment in the last two years. The survey is completely anonymous and will only be used to inform the ongoing improvements to the service.
