Eating well in Cheshire West and Chester

 

Partners across Cheshire West and Chester are using this questionnaire to find out about how easy you feel it is to eat well, to reach and maintain a healthy weight, and to access information and support to enable you to have a healthy diet.

The survey should take around 10 minutes to complete and you can quit at any time.

As a thank you for completing the survey, you will be entered into a prize draw to win a £25 Love to Shop voucher - there are four vouchers up for grabs! To enter, you will need to enter your email address at the end of the survey - if you don't wish to enter the prize draw you do not need to provide this information.
