Thank you for expressing an interest in joining the South or North Acton ward panel.

Prior to completing the survey I need to draw your attention to some important documents.

First is the terms of reference, these are essentially the rules of the panel. As part of the application you will be asked to confirm you have read and accept the TOR.

The second document is for your information and explains how we will use your information.

Once your application is completed and has been accepted you will be sent a copy of the TOR, privacy document and a copy of the ward panel community members handbook for the relevant ward.
