Have you used MindWell to support you in your job role? If so, we'd love to hear about your experiences.
Thrive by Design* and Volition work together to manage MindWell, the mental health website for people in Leeds, and we want to make sure it's meeting your needs. Your feedback is incredibly valuable and will help us improve the website for everyone.
There are X questions that are mostly multiple-choice tick box answers and a few questions where you can add comments. Once you have started the survey, you won’t be able to save it and come back to it later, so please make sure you have enough time before you begin.
How we will use the information you give us:
- Your feedback will be used by the MindWell team (Thrive by Design and Volition) to help us improve the website.
- Taking part in the survey is voluntary.
- We collect your views in digital format.
- We will share your feedback with our funders, the West Yorkshire Integrated Care Board. We may include quotes of what you tell us. We will not share or publish any personal information that may identify you as an individual.
- We will ask for personal information such as your name, email or phone number in some sections. You do not have to provide this information, however if you choose to, it will be kept on our secure internal system. Once we have looked at the information you have given us, it will be disposed of securely.
- All information is used and held under Articles 13 and 14 of the General Data Protection Regulation (GDPR). The full guidance on GDPR can be accessed here.
