1. Family Carers Survey - Introduction

We want to hear about your experiences of support for family carers in Suffolk. The information you give us will be used to assess what is currently working well for family carers and to identify any gaps in services for family carers. We want to know what works well, what could be improved and what's missing. All answers will be used anonymously so please do answer openly and honestly. If you want to withdraw any information you have given, please contact Helen Robinson on 01473 260354 (9am – 4pm Monday to Friday) or by email putting the word ‘survey’ in the title. This survey should take around 20 minutes to complete. The survey closes on Thursday 22 November 2018. Thank you.

1. A carer is anyone who cares, unpaid, for a friend or family member who due to illness, disability, a mental health problem or an addiction cannot cope without their support. The person cared for will be aged 18, or over for the purposes of this survey. Do you meet this definition of a carer?  *
