Access to dentistry for head and neck cancer patients
0%

A bit of context for this survey

At Macmillan we are exploring whether people with a diagnosis of head and neck cancer have adequate access to dentistry services.

To support this piece of work we are asking patients for information on the services they were offered at diagnosis, during treatment, and after treatment.

Thank you for taking the time to complete this survey - your insights are critical to help us to make a difference for people affected by cancer.

1. When were you treated for head and neck cancer? (please state the year)

2. Where were you treated? (please state the name of the hospital(s), and town/city

3. Before your cancer diagnosis, had you been accessing a dentist for routine check-ups/dental treatment?
