Survey for people with an Autism Spectrum Condition

1. Background

Healthwatch Wiltshire gathers feedback from local people about health and social care services. We are working with Wiltshire Service Users Network on this project.

We want to find out what people with Autism Spectrum Conditions and their carers think about health, care and support services.  We use the information to tell those who run services what could be improved or developed in the future.

We do not require names and the information you provide is confidential, except that anonymised quotes may be used.

Save and Continue later - If you want to save your answers and come return to the survey, scroll to the end and click 'Save and Continue later'. You'll be asked for an email address and the link to your survey will be sent to you to complete.

Please click continue to confirm you understand the purpose of the survey and consent to the use of the data as described above.
