This survey is for people with a visual impairment that is not fully corrected by glasses or contact lenses.
About the survey
We're Healthwatch Suffolk - your local NHS and social care champion. We use people's experiences to influence local standards of care and support from the NHS and social care services.
By completing the survey, you'll help us better understand what it's like to live with a visual impairment and to access NHS and social care services in Suffolk - including what's working well and what could be improved.
Your participation in this survey is completely voluntary. Your responses will be anonymous, the survey does not ask for your name or contact details. However, if you provide personal information (e.g names, dates of conversations) in your responses, then it may be possible to identify you. Healthwatch Suffolk will remove this potentially identifiable information from the data.
It is important to note that your comments may be shared with health and social care services, and leaders, to shape their work. By doing so, we hope to highlight how standards of care and support could be improved for people in Suffolk. We will also share experiences nationally with Healthwatch England so that it can use our local data to inform its work and influence national health and care policy.
Please do not complete the survey if you are not happy for people outside of Healthwatch Suffolk to have access to your comments.
The final report(s) about this survey for our website will contain example quotes. However, we will make the use of these quotes as anonymous as possible before they are published. Your personal questionnaire answers will not be shared with anyone outside of the Healthwatch Suffolk research team.
The survey will take you between 15 to 20 minutes to complete and you must be over the age of 16. Please confirm this below to proceed.
